• Please visit fsso.ca and click ‘Download PDF form’ to download the application form, fill and sign the form. You DO NOT need to fill the webform you find when you visit the site.
  Note – Please make sure to read the Terms and Conditions
• Signed application with a void check should be mailed to the Address: Funeral Support Services Organization (FSSO), 1306 Mowat Lane, Milton, ON L9T6C2
• Registration Fee payment Options
  • via eTransfer – send to info@fsso.ca. Please mention Principal Applicant Name in the notes
  • via Check – Make payable to ‘Funeral Support Services Organization’. Include the signed check with the application.
